Community Needs Assessment & Plan

A Community Plan is required to help Boards identify local priorities, assess community needs, engage partners, and implement effective, sustainable strategies that improve outcomes. Using the Results-Based Accountability framework, the plan establishes measurable indicators, examines factors affecting progress, and prioritizes strategies and funding to address the community’s most important needs. Plans should be reviewed and updated at least every three years, or sooner as local conditions warrant.
